NOTICE: Fillable Version of K-WC 1101-A Accident Report Available

As a convenience to our customers, the K-WC 1101-A Accident Report, formerly known as the Employer's Report of Accident, is now available in a fillable (interactive) PDF version. This version can be filled out online and saved to your computer, or downloaded first to your computer to use as a master copy and then filled out and saved with a different filename. You then print out and mail or fax the form to the Division; the Division does not accept e-mailed accident reports at this time. The second page contains instructions. We no longer include a page to fill out for your OSHA file, since the OSHA reporting requirements have changed since this page was added to our form as a convenience years ago. There is a web address in the instructions where you can find current OSHA reporting requirements.

K-WC 25, Workers Compensation Information for Kansas Employers & Employees, is once again available for download. Download here or go to our Forms and Publications: 2011 New Law page. This publication has been updated based on changes to the law effective May 15, 2011 with the passage of Substitute for House Bill 2134.

 

NOTICE: Workers Compensation Bill Available

The workers comp bill (Substitute for HB 2134) as signed by Governor Brownback can be found on the Forms and Publications page immediately under the Statutes & Regulations heading. This is the enrolled bill version, complete with strikethroughs and italics. The Division will have a cleaned version of the individual statutes in its new law book to be published this fall. Important: The statutes in this bill apply only to injuries sustained at work on or after May 15, 2011.

This website allows registered users to download claims information after submitting signed Request for Workers Compensation Records, Forms K-WC 97, 98, or 970, to the Division. Paper copies of the request forms are necessary due to signature requirements. Users need to register one time only.
