transition, some of these records may be temporarily unavailable. Please allow additional time for processing of records requests.

Hearings and Settlements
If you are an injured worker, attorney, insurance carrier or employer and you would like to file an application for hearing or settlement, please download one or more of the
Hearings and Settlements forms. Fax or postal mail only to number or address on form.
Do not email applications for hearings; emailed applications will not be accepted due to security concerns.
Requests for Records
