1099-G forms have been mailed to claimants who received unemployment benefits in 2014. If you did not receive a 1099-G you can request a duplicate copy online at www.dol.ks.gov/mail1099G.aspx . If you received a 1099-G and believe the amount shown is incorrect, you will need to send a written dispute to KDOL at: 1099 Inquiry, Kansas Department of Labor, 401 S.W.
