By state law K.S.A. 44-5,104, insurance companies and group-funded self-insurance plans providing workers compensation coverage to employers in Kansas are required to provide certain safety services to those they insure.

Accident Prevention Notice for Insurance Companies and Group-Funded Self-Insurance Plans
Insurance companies and group-funded self-insurance plans licensed to write workers compensation coverage in Kansas must file form
(Year-end Accident Prevention Report) with the Kansas Department of Labor annually after the close of the state fiscal year. The period to be covered by the report is from July 1 of the previous year through June 30 of the current year. This report must be received between July 1 and August 31, unless an extension has been requested and granted.
NOTE: The form is automated. Form K-ISH 28 must be submitted online and the Excel spreadsheet, if required, can be downloaded, completed offline, and then submitted online with the K-ISH 28. Effective 2011, we no longer accept hard copies of either the K-ISH 28 form or the Excel spreadsheet; also, we no longer accept homemade forms.
